This is a tester that I bought new, and it is the original discontinued bottle version of what is now called Opus V Woods Symphony.
100ml EdP (See photos for amount used).
Opus V is a woody floral musk with orris and oud dominating, and rose, jasmine, rum, and civet making guest appearances and complicating the overall impression. Quite lovely, and strange.
Of note, the cap works, but the inside part slides up and down; so you have to readjust it every time you open it in order to secure the cap.
